Application for Seafarer's Record Book and Identity Document Requirements

To get Zanzibar Seafarer's Record Book and Indenity Document,you must be submit the following Endosments Documents:

For National Seafarer's

  • Recent passport-sized photograph with white background (3.5 cm x 4.5 cm)

  • The Fee of $35 dollars for Seafarer's Book

  • The Fee of $25 dollars for Identity Document

  • Copy of Identity Document Passport

  • Copy of Identity Document National ID

  • Copy of STCW qualification Basic training

  • Copy of Letter of intent for familiarization

For International Seafarer's

  • Recent passport-sized photograph with white background (3.5 cm x 4.5 cm)

  • The Fee of $100 dollars for Seafarer's Book

  • The Fee of $150 dollars for Identity Document

  • Copy of Identity Document Passport

  • Copy of Identity Document National ID

  • Copy of STCW qualification Basic training

  • Copy of Letter of intent for familiarization
